Rescript

Use this form to request a new prescription for medication(s) you have previously been prescribed by CannaPlus+.

If you have not had a consultation with one of our doctors within the last six months, please book a follow-up consultation instead.

You will also require a follow-up consultation if you are requesting new medications / other changes to your treatment plan, or if you have never had a follow-up appointment with us.

If you already have prescription repeats available, you can request these by emailing a request to info@cannaplus.co.nz.

Not sure if you have prescription repeats available? Submit a rescripting request here and, if you have repeats at our dispensary, we’ll let you know (no rescripting fee will be incurred if you already have available repeats).
