This form lets you request a replacement prescription for an out-of-stock medication.

Important notes:

This form can only be used if you hold a valid prescription with available repeats through the CannaPlus+ Dispensary.

Submitting this form does not guarantee that a replacement prescription will be issued, as all prescriptions are always at doctor’s discretion.

Replacement prescriptions cancel the prior script, and will be for the same quantity as you previously had available of the out-of-stock medication.

For each out-of-stock medication, our doctors have selected the most comparable medications based on their expertise (options are listed in the form below). Replacements other than those comparable products or another medication you have previously received will require a follow-up consultation. Please do be aware that no two strains are perfectly alike, and different strains may have different effects.

When we issue a replacement prescription, you’ll be sent a payment link. If you don’t yet need the prescription, feel free to leave it for later! The prescription and payment link will remain valid for the usual length of time (7 days for Class B THC-containing oils, 90 days for other prescriptions).
